We have details of a SHOCKING INCIDENT that happened yesterday.

An adult film star named Stephen Hill went BONKERS at the production center of a Van Nuys porn company.

Here's how it all popped off. Stephen had fallen on HARD TIMES recently, and so the owner of the production facility allowed him to SLEEP on the set until he got back on his feet.

Well Stephen stayed there for SEVERAL MONTHS - and it didn't look like he was EVER GOING TO LEAVE. So yesterday, the owner along with 2 of Stephen's co-workers asked him to find some place else to stay.

Well that's when dude SPAZZED OUT. He pulled out a samurai sword and started HACKING FOLK.

One man, Herbert Wong, was killed and the other two were seriously injured.
